Section A: Excused Absences/Make-up Participation Points

Using the Attendance and Flexibility Consideration Rubric, I have determined that:a student with a disability may miss class for disability-related reasons beyond the number of allowed absences on the syllabus. Additional information is required.
The maximum number of excused absences without penalty permitted:
Additional details or exceptions to allotment: I expect students to contact me in advance of an anticipated absence. Absences from class periods (this is a seminar course; there are no separate discussion sections) are excusable on the basis of health-related need. I will exercise my own discretion if a student's required absences begin to seem significant enough to impede the student's ability to meaningfully participate in the course (this has never happened yet). In such a case, I will discuss a make-up plan. See below:
Plan for make-up participation points, if applicable (e.g. .submit additional blog posts on Canvas):So far I have not had a student miss enough class that I have felt the need to institute a make-up policy. If a student misses an entire week of class (under the terms of an accommodation arrangement that we have made together), I will ask the student to plan a meeting with me, over zoom, to go over the material for that week in some degree of detail. I am not inclined to assign extra work.
Please explain why absences beyond the number allowed in the syllabus is not a reasonable accommodation.

Section B: Deadline Extensions for Assignments

Important Notice to Students: The consequences outlined for the class for missing the deadline applies to the adjusted deadlines for the student using this accommodation.
Using the Deadline Extensions Consideration Rubric, I have determined that:students with a disability may have deadline extensions. Additional information is required.
Applicable assignments (check all that apply):homework assignments, papers
List any assignments that are not approved for deadline extensions

Please specify the expectations for deadline extensions.

Assignment Type(s)Canvas posts, essays
Timeframe for deadline extension (select one):The assignment(s) will be due within a certain number of business days of the original due date. Additional information is required.
Specify the number of business days: This is an arrangement I will make with the individual student.
Specify the revised due date:
If applicable, please detail the notification plan for requesting assignment extensions. In general, I expect notice within 48 hours of the due date to grant an extension. When the extension is granted as part of an SDS agreement, I want to be notified on the due date (i.e. I expect students not to simply miss their deadlines and tell me after the fact). I'm very accommodating and I am in nearly all cases (where the student needs special accommmodation) happy to take till just before grades are due, or to allow a student to take an incomplete.
Please explain why deadline extensions beyond the number allowed in the syllabus is not a reasonable accommodation.

Section C: Make-up Quizzes/Exams

I have determined that administering a make-up quiz/exam is:not applicable in the course.
