The following form must be signed and copies given to both the physician, psychiatrist/psychologist, other health care professional, or hospital, clinic, or agency providing medical and service records regarding your condition and to the Coordinator for Disability Services:
The Authorization form must also be signed by your parent or guardian if you are a minor.
